Job Description:

Under the direction and supervision of the RN. The CMT is responsible for preparing and administering proper medications. Is responsible for performing direct resident care such as ADL, ambulation, transfers, feeding and activities. Is responsible for adhering to pertinent nursing standards and Surrey Place policies. Performs duties and responsibilities in a manner consistent with our mission and values.Principle Accountabilities: 1. Performs medication delivery as follows: 15%a. Take medication report from previous shift and complete report for following shift.b. Administer prescribed medications to residents as medication certification defines. (NO sub , IM, IV med.)c. Pass all ordered po/prn medicationsd. Pass oral, topical, ophthalmic and inhalation medications.e. Document all medications administered to residents.f. Conduct regular narcotic counts.g. Reorder meds as needed. h. Notify nursing supervisor/or any potential problems i. Note changes in resident's condition and report them to nursing supervisor.j. Clean and stock med cart and med room. k. Receive and store meds from pharmacyl. Test resident's blood to determine glucose levels and report findings to R.N./L.P.N.2. Performs resident care as follows: 20% a. Position residentb. Assist with toiletingc. Applies ice packsd. Assists with transfers, ambulatione. Assist with bathing, oral hygiene, skin care etc.f. Takes daily weightsg. Applies TED hose h. Applies simple oxygen and change tanks i. Perform VS and apical pulsesj. Assist with proceduresk. Report pertinent resident changes to nurse l. Plans and carries out activities. 3. Delivers resident's meals and assists resident with feeding 10% a. Check meals for correct diet prior to serving b. Assist with room trays as neededc. Assist in MDR as needed.d. Passes snackse. Passes water on unit4. Documentation 10%a. Records input/output, consumption, VS etc.b. Records all resident's chargesc. Mark laundry to identify residents clothingd. Utilize and update Kardexe. Complete monthly summary and dementia rating scale under supervision of R.N.5. Equipment 10% a. Maintain cleanliness of equipmentb. Assist in inventory of suppliesc. Stock linen carts d. Restock resident rooms e. Operational knowledge of all equipmentf. Disinfects tubs/showers after each use6. Responds to STAT situations and protocols as appropriate 2%7. Assists with admission, discharge and treatments 15%8. Sorts laundry appropriately 1%9. Environment 3% a. Keeps resident rooms, nursing station, halls etc. tidyb. Report maintenance repairsc. Empty trash in resident rooms.10. Rounds on residents every 2 hours 5%11. Attend inservices/education programs to maintain required continuing education 1%12. Participates in OPOC, restraint rounds. 1%13. Assist and conduct activities 5% a. Chauffeurs licenseb. In-house and off-site activities.14. Performs other work duties as assigned. 2%High School Diploma or equivalent. CNA certified and CMT certification required. Basic math, writing and reading skills required. The assessment, care and treatment provided by the staff will be consistent with the specific age related needs of the patient. The staff is competent to care for patient age groups including the adult and geriatric patientEmployee is required to stand and walk frequently (34-66% of the time). Employee must lift/carry frequently Employee may be required to lift over 100 lbs with assistance. Employee is frequently required to bend, squat, kneel and reach forward occasionally (1-33% of the time). Employee is required to simple grasping, pushing/pulling and fine manipulation. Employee is occasionally exposed to the following environmental conditions: Temperature changes, dust, noise, fumes, electrical hazards, and CRT terminal work. Employee may be exposed to blood/air borne pathogens. Verbal and cognitive abilities are necessary. Motor coordination and manual dexterity are required. Speech, hearing and visual acuity are essential. Work shifts may vary to meet specific patient care demands.
