Description Responsible for the provision of social work/discharge planning services to patients in need of these services. Works in active partnership with the RN care manager, the disease manager and other disciplines to develop and implement a well-coordinated personalized plan to address patient needs across the continuum. The social worker assesses plans and facilitates options and services to meet an individual's unique and complex needs. This is accomplished by addressing both the individual patient's medical and psychosocial needs, as well as the resources available. There is a continuing commitment to maximizing the quality of life for each patient and enhancing community health. Performs duties and responsibilities in a manner consistent with the hospital mission, values and NASW Code of Ethics. Principle Accountabilities: % of Time Spent 1. Based on nursing and care management departmental screening processes, patients will be identified for appropriate social work referrals as early as possible. 5% 2. In conjunction with the RN care manager and disease manager, a coordinated plan be implemented based on care needs identified from preadmission or admission to discharge assuring each patient's identified needs are addressed. Identifies appropriate education needs of patients utilizing in house and community resources. Facilitates post discharge arrangements. Oversees that they are carried out in a safe and appropriate manner. Communicates plans, changes to healthcare team members. 15% 3. Completes psychosocial assessment consistent with department guidelines. Able to define and address patient continuum of care needs. requires knowledge of human development and behavior, of social, economic and cultural institutions and their interaction as part of the patient/family system. 15% 4. Works in collaboration with the psychosocial department and engages their services as defined by policy. 10% 5. Provides brief, targeted therapeutic interventions for identified problems. Focus on the adaptive capacities of patients/families/caregivers with the goal of preventing maladaptive behavior and enhancing recovery. Appropriately assesses situations of abuse, neglect, domestic violence and sexual assault and refers them to the psychosocial department per policy. 10% 6. Collaborates with all disciplines involved in formulating an individualized plan of care for each patient. Serves as an advocate for patient/family and encourages patient/family participation in the plan of care and discharge plan. Ensures that the patient discharge is timely, appropriate, and safe incorporating the best use of resources that the patient may need in the community. 10% 7. Facilities an optimal transition from acute care to community so that gaps in care are minimized. 5% 8. Is knowledgeable of hospital and community resources and ensures that appropriate resources and services are provided in the most appropriate setting or level of care. Develops and maintains professional relationships with community resources and insurance providers. 5% 9. Maintains factual, accurate, organized, complete, confidential, legible documentation according to department policy. 10% 10. Maintains knowledge base through continuing education (per department standards) and applies knowledge to current practice of social work and health care. 5% 11. Participates in department PI activities to ensure and enhance the quality of patient care and social work services. Assists with compliance of federal agency quality indicators when appropriate. 5% 12. Participates in Hospital/Department HICS (Emergency Response) Training, Drills and responses per hospital policy. 3% 13. Performs other work duties as assigned. 2% Total 100% Education Masters degree in Social Work. Experience* Experience in a hospital or skilled nursing facility preferred.* Employee must be able to communicate orally, in writing and electronically.* Employee must be able to listen and to understand and explain complex situations.* Must possess group work skills and collaboration skills. The assessment, care and treatment provided by the social worker will be consistent with the specific age related needs of the patient. The social worker is competent to care for patient age groups including the neonates/infants, children, children, adolescents, adults and geriatric adult patient. Working Conditions* Employee is required to sit frequently and walk occasionally. Employee must lift/carry a light (1-20 lbs.) load occasionally. Employee may be required to lift over 20lbs. Employee is occasionally required to bend, squat, crawl, kneel, climb, lift waist to overhead.* Employee is frequently required to reach forward and reach overhead. For repetitive action, employee is required to use both hands for simple grasping and pushing/pulling. Employee is occasionally exposed to electrical hazards and CRT terminal work. (Occasionally: 1-33%, Frequently: 34-66%, Continuously: 67-100%) The most significant of duties are included but this does not exclude occasional work assignments not mentioned. Job Relationships: Member of the Social Service Department who interacts regularly with patients, families, physicians, visitors, employees and employees of community resources/agencies.
