Job Description Towne U.S. LLC is seeking a travel Home Health Physical Therapist for a travel job in St. Louis, Missouri. Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: 03/23/2026 - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Physical Therapist (PT) – Home Health We are seeking an experienced and independent Physical Therapist (PT) to provide skilled home health services to patients in their place of residence. This role offers the opportunity to deliver one-on-one, patient-centered care while promoting safety, mobility, and independence within the home environment. The PT will evaluate patients recovering from illness, injury, surgery, or hospitalization and develop individualized plans of care designed to reduce fall risk, improve functional mobility, and prevent rehospitalization. This position requires strong clinical judgment, autonomy, and effective communication skills. Key Responsibilities: - Perform comprehensive home health evaluations and establish individualized plans of care - Provide skilled interventions focused on gait training, balance, strengthening, endurance, and functional mobility - Conduct home safety assessments and recommend environmental modifications - Develop and progress individualized home exercise programs - Educate patients and caregivers on fall prevention and safe mobility strategies - Complete timely documentation including OASIS assessments, progress notes, and discharge summaries - Collaborate with interdisciplinary team members including nursing, OT, SLP, and case management - Manage daily scheduling and travel between patient homes Ideal Candidate: - Active PT license in practicing state - Home health experience preferred - Proficiency with OASIS documentation - Strong time management and organizational skills - Comfortable working independently in a community-based setting This is an excellent opportunity for a PT who values autonomy, meaningful patient relationships, and delivering high-impact care that supports aging in place and functional independence.
