Job Description Job Description UCP Heartland is seeking compassionate and committed PERSONAL ASSISTANTS (Direct Care Providers) for PART-TIME shifts. UCP Heartland offers a positive, team driven, work environment at $20.00/Hour MEHLVILLE (SOUTH COUNTY) THURSDAY 3 PM - 11 PM FRIDAY 3 PM - 11 PM SUNDAY 7 AM - 3 PM (THIS IS A SET 24 Hours Weekly SCHEDULE) As a PERSONAL ASSISTNAT/DSP (Direct Care Provider) you will perform the following responsibilities: Provide physical assistance with daily tasks, assist with personal hygiene and facilitate fun activities. Follow medical orders and administer medications. Assist with ADL's; Lift and Transfer (includes ceiling lifts, repositioning in wheelchair, etc), Adaptive transportation. Shift Documentation using Set-Works and general housekeeping. MINIMUM EMPLOYMENT REQUIREMENTS: High School diploma or its equivalent (GED). Valid Drivers’ license Clean Driving Record and successfully completes and maintains assigned drivers training A MINIMUM OF ONE WEEKEND SHIFT PER WEEKEND Experience supporting individuals with disabilities or in a customer service field Ability to take direction and work with minimal supervision. Ability to consistently work in a professional, cooperative manner with all Agency clients and personnel. Good written and oral communication skills Computer Proficiency THIS POSITION DOES REQUIRE RELIABLE TRANSPORTATION, VALID DRIVER'S LICENSE WITH CLEAN DRIVING RECORD!
