Job Description:
The Home Health Occupational Therapist evaluates, plans, and
administers comprehensive occupational therapy modalities/regimens
to patients in their place of residence for the purpose of
developing and restoring function through rehabilitative
occupational therapy as prescribed by a physician.Principal
ResponsibilitiesProvides initial and ongoing assessment to
determine level of function, and any other assessments as required
by federal guidelines for patients who have debilitating conditions
improve the functions of performing everyday tasks in the
home.Establishes an in-home treatment plan in coordination with the
physician and other care team members to develop the daily living
skills of their patients and the basic motor functions of patients,
as well as to compensate for any loss of function that patient may
be experiencing.Responsible for planning, evaluation, and treatment
implementation of an assigned caseload.Provides quality,
personalized, evidence-based care, and proven interventions of
Occupational Therapy.Educates, collaborates with, and conferences
with patients, families, and other health care providers; making
recommendations for referrals to specialists as needed.Supervise
Occupational Therapy Assistants and other home health staff as
appropriateCompletes required documentation to complete the
patients visits and episodes as required by federal, state and
payor guidelinesInstructs patients and family/significant others in
home programs and activities of daily
livingEducation/ExperienceSuccessful completion of a program
approved by the National Board for Certification in Occupational
TherapyOne year experience in hospital or Home Care Setting,
preferred .BS degree or greater in occupational
therapyLicenses/CertificatesCurrently Occupational Therapy license
by the State of IllinoisCurrent certification by the NBCOT is
preferred.AHA CPR certification requiredReliable means of
transportation, valid IL drivers license and vehicle liability
insurance requiredKnowledge, Skills and AbilitiesStrong
communication skills, flexibility, and
professionalismSelf-direction with a strong sense of team
commitmentStrong organizational and time management skillsPersonal
care component on a small percentage of patientsWorking
EnvironmentThis job operates in a variety of settings, including
but not limited to professional office environment, patients homes,
and car Routinely uses standard office equipment such as computers,
phones, photocopiers, filing cabinets and fax machines.Must be able
to function in a wide variety of environments which may involve
exposure to allergens and other various conditionsPossible exposure
