Currently seeking skilled and driven Wound Care Nurse to welcome
our ever-growing team!

communities we serve.
The Wound Care Nurse is responsible for the prevention,
investigation and monitoring of wounds. Also responsible for
keeping records of trends in resident wounds.
Essential Functions & Responsibilities:
--- Assess the unique needs of each patient to determine
appropriate treatment plans
--- Educate patients and families on best practices to promote
healing and wellness
--- Track treatments and patient progress according to HIPAA
--- Coordinate with supervising physicians to ensure smooth
--- Provide excellent nursing care to patients suffering from
--- Ensure optimum patient care delivery in wound care nursing
--- Educate and counsel patients and their families on wound care
processes and issues.
--- Monitor and treat all types of skin problems of residents
including open wounds, scars or skin infection and skin rashes or
reactions. Preventing the skin breakdown.
--- Coordinate with Medical Director and DON on wound care of
--- Acts as a point of resource to staff and the family of
residents concerning wound care issues, news and education.
--- Responsible for implementation and overseeing of policies and
procedures involving wound care.
--- Identify areas in which education of staff is needed and work
with Nurse Management staff and DON to implement in servicing.
--- Current state licensure as a nurse- RN LPN -
--- CPR and basic life support (BLS) certifications
--- Certification and prior experience in wound care.
--- Wound Care Certification required
--- Nursing: 1 year (Required)
--- Must be able to read, write, speak and understand the English
--- Must possess the ability to make independent decisions when

--- Must be CPR certified.
