Job Description:

**Please Note: This is a hybrid position. This position sits in Chesterfield, Missouri** -Overview: The Social Worker, as part of the Population Health team, intervenes with patients who have medical needs. In addition, the Social Worker assess patients psychosocial needs and connects patients to community based resources, facilitate patient access to community based services and monitor referrals made to community based organizations. This role receives referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, nurses, advanced practitioners and other members of the care team). -Qualifications:

• Education: Masters in Social Work, Required, or has satisfactorily completed all requirements for the MSW
• Licensure: Current license in Social Work, master's level or above, in the state of employment
• Minimum Experience: Minimum 2 years' experience in acute care hospital setting, adult experience preferred
• Certification/Registration: BLS (CPR) at hire date, required, or within 90 days of hire
