Job Description:
The nurse aide provides compassionate care to the terminally ill
by performing assigned tasks of personal care for each patient
according to the Home Health Aide Assignment Sheet and under
established St. Luke's Hospice policies and procedures. Personal
care includes, but is not limited to bathing, hair care, mouth
care, vital signs, socialization and assistance with meals.

customers. -Principle Accountabilities: Perform assigned tasks of
personal care for each patient: Demonstrates the ability to
appropriately follow the assigned Home Health Aide Assignment Sheet
by providing appropriate personal care including, but not limited
to: Bathing, Hair care, Mouth care, Socialization, Assistance with
meals Performs procedures, treatments and exercises as prescribed
Documents accurately and completes assignments in a timely manner
Identifies changes in patient condition and reports to RN Case
Manager Communicates clearly with staff, patients, families etc.
Maintains confidentiality at all times Safety: Maintains clean,
safe environment while performing tasks Identifies and reports any
safety needs or unsafe condition Other job related duties: Attends
and participates in IDT (Interdisciplinary Team meetings)
Participates in QI (Quality Improvement) activities Observes Policy
and Procedures Maintains proper dress code and personal hygiene Has
flexible work habits and interacts effectively with others
Education: Attends educational programs to enhance professional
growth Maintains current CPR certification Maintains current PPD
Maintains Certified Nurses Aide Certification in good standing
Performs other work duties as assigned. Scheduling Expectations:
Employee is expected to work a set schedule Monday through Friday.
Employee is expected to give enough notice for vacations to be
granted. It is required to give at least a 2 hours notice when
calling in sick when at all possible. In the event of an emergency,
please notify your supervisor immediately. General Requirements:
Maintains acceptable attendance record per facility policy Meets
annual compliance guidelines: Maintains confidentiality and support
the basic right of privacy Attends annual compliance training
Demonstrates safe workplace behaviors for self, co-workers and
patients Uses appropriate body mechanics Use of facility provided
safety devices/equipment Attends annual safely training Completes
annual Employee Health requirements (PPD, etc.) Provides Patient
Education Provides information and education to patients, families
and visitors as appropriate to position -High School diploma or
equivalent, or four years of direct experience in home care.A
minimum of one year of Homecare experience is preferred. 2 years of
hospital, nursing home or public health nursing is preferred.
Satisfactory completion of the State Home Health Aide Competency
Examination will be completed during departmental orientation. Must
be able to read, write, and communicate well. Must be able to
document patient care on appropriate agency form. The assessment,
care and treatment provided by the Home Health Aide will be
consistent with the specific age related needs of the patient. The
Home Health Aide is competent to care for patient age groups
including the geriatric patient.Employee is required to sit
continuously and walk occasionally. Employee must lift/carry a
light (1-20 lbs.) medium (21-50 lbs.) and heavy (50 lbs. +) load
frequently. Employee may be required to lift over 100 lbs. with
assistance. Employee is frequently required to bend, squat, crawl,
kneel, climb, lift waist to overhead. Employee is occasionally
required to reach forward and reach overhead. For repetitive
action, employee is required to use both hands for simple grasping
and pushing/pulling. Employee is occasionally exposed to electrical
hazards and CRT terminal work. (Occasionally: 1-33%, Frequently:
34-66%, Continuously: 67-100%) Employee is frequently to
continuously alone with patient and must be able to handle
emergencies without assistance. Employee must be able to assist
falling or fallen patient; must be able to perform CPR alone.
Employee may be required to drive at night/and or in hazardous
conditions; driving requires the use of at least one foot, must be
able to follow written/verbal instructions and read a map. Employee
must possess the ability to make accurate and appropriate judgments
in assessing patient needs through on-site assessment and verbal
reports. Employee has regular contact with agency staff members,
physicians, hospital staff and general public. Contact frequently
involves problem resolution. Employee regularly handles
confidential information. Employee may be expected to document care
and services into an electronic medical record. If utilizing an
electronic medical record employee is required to synchronize data
at the beginning and end of each work day. This may be done from
the employee's home, a public domain or from the office. The most
significant of duties are included but this does not exclude
occasional work assignments not mentioned or developmental duties.
