Performs the primary function of a Registered Professional Nurse in assessing, planning, implementing, educating and evaluating a designated group of patients in accordance with all established Nursing Standards and Hospital Policies. Is responsible for managing all assigned personal supplies, and equipment on the unit and for promoting teamwork with all members of the healthcare team. Performs duties and responsibilities in a manner consistent with our mission and values.




PRINCIPLE ACCOUNTABILITIES IN OPR:




Performs initial patient assessment. 20%

Coordinates all aspects of patient care following the Nursing Process using age specific knowledge relevant to the patient served. 15%

Assures all OPR Phase II orders are processed /carried out in a timely manner. 15%

Documents all aspects of patient care. 15%.

Plans/participates in patient discharge from OPR. 15%

Participates in checking/maintaining equipment/supplies and the physical environment. 5%

Committee participation/Educational Activities/Special Projects.1%

Supervises other team members in care delivery. 8%

Performs clerical/computer tasks. 5%

Performs clerical/computer tasks (exclusive from the Perioperative Record) 3%

The patient care provided will be consistent with the specific age related needs of the patient. Nursing Units may provide care to neonates/infants, children, adolescents, adults and geriatric adults.




ASSESSMENT:

A. Collecting data from the patient/family and other health care providers by interview, observation, inspection, palpation, reports and records.

B. Demonstrating ability to recognize the patient's emotional, safety, psychosocial needs and taking appropriate action.

C. Demonstrating ability to gather all necessary information for the patient'sdesignated surgery (i.e. labs, EKG, chest x-ray, History and Physical, pre-operative interview, physician's orders).

D. Organizing data so it is complete, accurate, and accessible and yet remains confidential.

E. Presenting any abnormalities to anesthesia personnel/surgeon responsible for the patient.

F. Ability to assess the status of /provide care to surgical patients using age specific criteria.




PLANNING:

A. Demonstrating ability to care for multiple patients.

B. Setting goals based upon assessments.

C. Setting priorities for appropriate nursing actions.

D. Individualizing goals based on patient'sneeds and nursing diagnosis.

E. Providing the patient with adequate physical/emotional preparation for surgery. INTERVENTION:

A. Implementing planned nursing care in a safe, competent and skillful manner.

B. Assisting with or performing procedures/treatments as outlined in physicians orders, protocols, or nursing standards.

C. Upholding the dignity of the patient/family reflected through nursing decisions and actions.

D. Demonstrating organizational skills in all aspects of patient care, using time effectively.

E. Administering medications safely and monitoring patients appropriately. This includes:




1.Maintaining awareness of action, side effects, contraindications and incompatibilities.

2.Administering medications consistent with Nursing Standards.

3.Identifying patient and verifying accuracy of medication, dose, route, and patient allergies prior to administration.




4.Exhibiting accountability for, documenting and reporting effects of drugs and intravenous therapies.

F. Administering I.V. fluids/blood products according to Hospital/Department/Unit standards in a safe, skillful and timely manner. G. Documenting pertinent data in a factual, concise. and legible manner. H. Identifying and reporting abnormal lab results, EKG's and chest x-rays and documenting same.

I. Understanding and applying pre-operative anesthesia requirements. J. Ability to perform the following specific functions:




1.Manage the airway of adult and/or pediatric patients.




2.Apply knowledge of anesthetic agents, techniques, actions and interactions to patients of all ages.




3.Management of the patient during altered states of consciousness.




4.Monitoring the status of patients receiving epidural/spinal anesthesia.

K. Giving a thorough/concise report to the division nurse or accompanying the patient when necessary per Unit Standards.




EVALUATION/REASSESSMENT:

A. Evaluating patient's response to therapies carried out both on physical and psychological basis.

B. Evaluating patient'sresponse to comfort measures including all PRN medications given.

C. Documenting reassessments and reporting those requiring new interventions in a timely manner. D. Adjusting the plan of care based on changes in the patient'scondition.

E. Evaluating the patient'sreadiness for transfer to the OR or appropriate area, noting that all requirements have been met (i.e. anesthesia requirements, consent form, hair removal, preps, scrubs, pre-operative medications, patient belongings secured, etc.).

F. Utilizing appropriate discharge criteria for the Minor Operating Room and Ophthalmic Laser Room patients.

G. In Outpatient Recovery, determining patient'sreadiness for discharge using the Phase II discharge criteria or admitting the patient to a division if necessary.




TEACHING/ DISCHARGE TEACHING

A. Clearly informing the patient/family of information that is appropriate to their level of understanding.

B. Incorporating teaching in all aspects of care from the pre-operative phone call through discharge instructions.

C. Planning for discharge measures to meet the patient'sassessed needs.

D. Evaluating the effectiveness of teaching and the practicality of the plan through feedback from the patient/family.

E. Documenting teaching/discharge plan and the patient'sunderstanding of these instructions. UNIT SPECIFIC

STANDARD FOR ENVIRONMENTAL SAFETY: THESE JOB DUTIES REQUIRE ONGOING VALIDATION FOR COMPETENCY, YEARLY PARTICIPATION IN PROGRAMS ON STANDARD PRECAUTIONS, BLOOD BORNE PATHOGENS AND FIRE AND SAFETY ARE REQUIRED. BLS HEALTHCARE PROVIDER CPR COURSE IS REQUIRED EVERY OTHER YEAR. IF UNABLE TO DO SO FOR MEDICAL REASONS, A PHYSICIAN'S STATEMENT IS REQUIRED. HOWEVER, INDIVIDUALS ARE STILL REQUIRED TO VIEW THE CPR VIDEO AND PASS THE WRITTEN TEST.



Education:

Current licensure as a Registered Nurse in the state of Missouri Required.

Nursing Diploma or Associates in Nursing Required.

BSN or completion of BSN within 5 years of hire date is required.
Experience:

Current licensure as a Registered Nurse in the state of Missouri. Two years Med/Surg Experience required.



Working Conditions:



Employee is required to walk continuously, sit and stand occasionally. Employee must lift/carry a light load (20# max) or medium load (50# max) occasionally and rarely (51-100#) with assist. Employee is occasionally required to bend, squat, kneel, climb or lift from waist to overhead. For repetitive action, employee is required to grasp, push/pull and fine manipulation. Employee is occasionally exposed to the following environmental conditions: fumes, radiation, electrical hazard and computerl work. Employee is frequently exposed to blood borne pathogens. May be required to work in other area of the Surgical Services Department. May be needed to work other shifts. (Occasionally: 1-33%, Frequently: 34-66%, Continuously: 67-100%)
