Overview: The Population Health Navigator works in active partnership with the patient, physicians, hospital, skilled nursing facility, office and virtual staff to develop and implement a well-coordinated and innovative discharge plan that ensures appropriate progression of care, length-of-stay and discharge disposition during a hospital and post-acute care stay. This position is integral to Mercy's efforts to provide high-quality patient care and implement successful care delivery methodologies needed for alternative payment models and value-based care. Qualifications: * Experience: Experience in Case Management, Utilization Management, or Managed Care. * Required Education: RN with a BSN or equivalent experience required. Current Missouri, Arkansas, and Oklahoma RN licensure required. * Certifications: Case Management certification preferred. * Other: Strong organizational, planning and problem-solving skills. Highly knowledgeable in Case Management and Utilization Management practices/protocol and managed care delivery systems. Excellent customer relations, communication, and organizational skills. Experience using data systems and Electronic Medical Records (EMR).
