The Hospice RN Case Manager will steward compassion through care plans for patients during the end of life process while working with an interdisciplinary team to ensure a wealth of resources are provided for each patient and family before, during, and after the end of life.Life is better with compassion!Patients and families near the end of life should have a caregiver committed to the art, science, and skill of delivering compassion.Death is a meaningful experience for patients and loved ones.
